Introducing the Grata Partner Portal.

The Grata Partner Portal shows you where each referral stands, how many got into care, and how fast — plus the one or two people this week who need a nudge from someone they trust.
The Overview, as a referral coordinator sees it. Every figure on this page is illustrative.
The referral today
01
You find out how someone is doing by calling around, or by running into them.
02
When leadership asks whether the referral relationship is working, the honest answer is that nobody has the numbers.
03
The patient with the wrong phone number is someone you could catch in a hallway — if anyone told you.
Inside the portal
Four things the portal does that a fax confirmation never could.
Every patient you have sent, with their current status — outreach in progress, first visit scheduled, treatment started, actively engaged.
Also in there
Weekly, monthly, or quarterly — a short aggregate summary of how your referrals are doing. Off by default; you pick the cadence.
Sign-in is a code sent to your work email. There is no password in the product to reset, share, or write on a sticky note.
No EHR integration, no VPN, no IT ticket, no cost. It is a web page your browser already trusts.
HIPAA and 42 CFR Part 2. Every view of a patient record is logged as a disclosure.
Getting access
01
Tell us your organization and your sites. Grata sets it up and maps it to your referrals — there is no self-serve signup, on purpose.
02
One form, one email each. They click the link, confirm their name and job title, and they are in. No IT ticket, no accounts to provision.
03
Your first referral appears on the roster the same day. Outcome metrics appear as soon as the first cohort is large enough to report safely.

No. The portal is free to referral partners, like everything else in the referral relationship — no fees, contracts, or revenue-share arrangements.
No. There is nothing to install and nothing for your IT team to approve. It is a website you sign into with a code sent to your work email, so most teams are up in an afternoon.
Yes, within the rules. Aggregate outcomes for your organization identify nobody. Patient-level detail appears only where that patient has specifically authorized sharing with your organization; everyone else appears as a row you can see but not open, with no reason given for why.
No. You see status, dates, and whether care happened — a first visit completed, treatment started, a visit missed. You never see notes, diagnoses, medications, or lab results, and there is no way to write to the record.
Whoever sends referrals and whoever asks how they went. Day to day that is usually care coordinators, discharge planners, and case managers; add a director or medical director on the shared dashboard link.
Existing partners are being moved over in waves. Ask for your workspace and we will tell you where your organization sits in the queue.
Ask for a workspace and we’ll set it up for your organization — or take 30 minutes with us first and we’ll walk through it with your team’s own referral history on screen.
